What will happen at my appointment?

You will meet in person with the doctor for an evaluation during which she will review and discuss your reported conditions with the end goal of assessing if medical cannabis may help in the treatment of your specific medical condition or ailment

Do I have to be concerned about any privacy issues if I receive an evaluation or recommendation?

No

Just like any doctor visit, yours is protected under the privacy laws of HIPAA (Health Insurance Portability and Accountability Act).

What are the qualifying medical conditions?

Patients are eligible to receive a certification for low-THC cannabis or medical marijuana if they suffer from the following conditions:

  • Cancer
  • Epilepsy
  • Glaucoma
  • HIV
  • AIDS
  • Post-traumatic stress disorder (PTSD)
  • Amyotrophic lateral sclerosis (ALS)
  • Crohn’s disease
  • Parkinson’s disease
  • Multiple sclerosis (MS)
  • Medical conditions of the same kind or class as or comparable to those above
  • A terminal condition diagnosed by a physician other than the qualified physician issuing the physician certification
  • Chronic nonmalignant pain caused by a qualifying medical condition or that originates from a qualifying medical condition and persists beyond the usual course of that qualifying medical condition
What are the state residency requirements?

For online applications:

– The Registry is connected to the Florida Department of Highway Safety and Motor Vehicles’ (FLHSMV) demographic database. If there is a match to the information on your profile, your state residency status can be automatically added to your application and approved.

For paper applications OR if there is no match in the FLHSMV system for state residency status, applicants must provide proof of Florida residency as detailed below, per Florida law:

– Permanent adult Florida residents must submit a copy of a valid Florida driver license or Florida identification card.

– Seasonal Florida residents who do not possess a valid Florida driver license or Florida identification card must submit a copy of two of the following documents:

– A deed, mortgage, monthly mortgage statement, mortgage payment booklet or residential rental or lease agreement.

– One proof of residential address from the seasonal resident’s parent, step-parent, legal guardian or other person with whom the seasonal resident resides and a statement from the person with whom the seasonal resident resides stating that the seasonal resident does reside with him or her.

– A utility hookup or work order dated within 60 days before registration in the medical use registry.

– A utility bill, not more than 2 months old.

– Mail from a financial institution, including checking, savings, or investment account statements, not more than 2 months old.

– Mail from a federal, state, county, or municipal government agency, not more than 2 months old.

– Minor patients must submit a certified copy of a birth certificate or a current record of registration from a Florida K-12 school and a copy of the minor patient’s parent or legal guardian’s valid Florida driver’s license or identification card.

For all forms of residency, the name and address on the document(s) provided must match the name and address provided in the application.

Do I need to bring medical records to my appointment?

No.

However any medical record you may have related to your claimed  qualifying condition or ailment will benefit your case by helping the doctor make a more accurate diagnosis. This include:

  • Actual medical records
  • MRI or X-Ray reports
  • Prescriptions
  • Medications
  • Other related documents.
How much does a Florida Medical Marijuana card cost?

$75

A patient or  designated caregiver is responsible for the cost of each visit to their medical cannabis doctor. Refer to our pricing for details.

Once the recommendation is issued by the doctor you can obtain your Registry ID Card at the cost of $75, paid annually to the Florida state Office of Medical Marijuana Use.

If my application is approved, where can I buy medical marijuana in Florida?

Medical marijuana may only be purchased from licensed dispensaries, also called Medical Marijuana Treatment Centers. For an up to date list, Click here for a full list of licensed dispensaries across Florida.

A patient may choose to buy their recommended medical marijuana product at the dispensary of their choosing, similar to how a traditional medical prescription can be filled by any pharmacy of the patient’s choice. Common dispensary choices are

What is the status of my Medical Marijuana Use Registry Card Application?

The Medical Marijuana Use Registry is linked to the Florida Department of Highway Safety and Motor Vehicles’ (FLHSMV) demographic database to make the ID card application and approval process more efficient. Using the FLHSMV sourced data will automatically approve the photo and proof of residency, and may cut down on processing time. Online applications take an average of 10 business days for approval.

If you are submitting an application by mail, please all an additional 3-5 days for postal delivery of your application.

Patients and caregivers can check the status of their application by calling 800-808-9580 or through the Medical Marijuana Use Registry.

How do I renew my ID card?

Renew 45 days prior to the card expiration date

To maintain an active Medical Marijuana Use Registry identification card, a patient and/or caregiver must annually submit a renewal application and any required accompanying documents to the department forty-five (45) days prior to the card expiration date.

Patients and caregivers can submit a renewal application either online or by mail.

What methods of payment do you accept?

Can I buy medical marijuana right after my appointment?

Depends on your residency status once you are approved by the doctor you will be guided through the process to enter or update your profile in the official State Use Registry

Permanent Residents

You will be provided a temporary identification on-site on the same day allowing you to go to any dispensary in Florida to purchase medication.

Seasonal Residents

The Department of Health takes between 5-10 business days to review your information. Once reviewed, you will receive an approval email from the DOH that will allow you to purchase medication.
